Support Coordination
ClarkCare provides support coordination services for individuals with intellectual and developmental disabilities who are qualified for services through the Division of Developmental Disabilities. Our Support Coordinators assist individuals and family members in obtaining the supports and services that best meet the needs and preferences of the individual. We utilize a person-centered planning process to distinguish support needs, planning goals, and outcomes, and then we apply this knowledge to develop and maintain an individualized service plan.

ClarkCare is pleased to provide Support Coordination Services to individuals residing in Atlantic, Camden, Cumberland, Gloucester, Burlington, and Salem counties.

ClarkCare is recognized as a “released agency,” meaning we have been granted authorization by the Division for our SC Supervisors to approve our service plans directly. This status indicates that ClarkCare no longer needs to submit service plans to the Division for review and approval, as we have met the required minimum standards for delivering high-quality service plans.
