ClarkCare provides support coordination services for individuals with intellectual and developmental disabilities who are qualified for services through the Division of Developmental Disabilities. Our Support Coordinators assist individuals and family members in obtaining the supports and services that best meet the needs and preferences of the individual. We utilize a person-centered planning process to distinguish support needs, planning goals, and outcomes then apply this knowledge to develop and maintain an Individualized Service Plan.
ClarkCare is proud to offer Support Coordination Services to individuals living in the following counties: Atlantic, Camden, Cumberland, Gloucester, Burlington, and Salem.

ClarkCare is a “released agency” which means that we have received authorization from the Division to have our SC Supervisors approve our service plans. This signifies that ClarkCare does not have to submit service plans to the Division for review and approval because ClarkCare has met a minimum standard for delivering quality service plans.
